Root cause

We had no control requiring re-verification of a supplier when that supplier changed its upstream source. Our distributor, Cascade Gear Supply, switched its manufacturer in March 2026. Because our purchase approval process checked only that the distributor was on our approved list, the change passed unnoticed and we continued listing under records that no longer traced to the actual manufacturer.
